Registered Nurse Navigator

The practice of the Registered Nurse Navigator is based on a wide range of knowledge, experience and research and has direct impact on patient outcomes in the assigned area within the facility and with the outside providers. The Nurse Navigator performs nursing processes which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well-being; and executing medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider. The nurse's primary commitment is to the patient, whether an individual, family, group, or community (ANA Code of Ethics for Nurses).The Nurse Navigator works closely with providers, other healthcare professionals and team members, internal or external services and community agencies in the coordination of patient care, which is timely, appropriate, of high quality and cost effective. Also, works closely with the Care in the Community Mid-level and provides following the consult request that are not available within the Salisbury VA Health Care System (VAHCS) or services that cannot be provided at the Salisbury VAHCS in a timely manner. The Nurse Navigator performs the following responsibilities and duties:
Initiates electronic or paper treatment plans depending on services requested for all faxed requests. This includes all Request for Services (RFS). Creates a new order / consult for all RFS if clinically indicated and will defer all care greater than 365 days back to the Community Care Provider, Mid-level or Primary Care Provider via treatment plan and supporting documentation. Coordinates VA and Care in the Community to ensure that high quality health care is provided. Ensures that medical documentation is received in a timely manner Scans records, complete, close and discontinue consults. Manages all Durable Medical Equipment (DME) from the community. Nurse Navigator will receive DME request from the community provider with supported documentation and create a treatment plan for Community Care provider to place DME consult. Coordinates with Care in the Community post-discharge needs, including prosthetics (DME, home oxygen), home health, etc. to ensure that Veterans discharged from Care in the Community receive appropriate medical supplies in a timely manner. Provides professional assessment, coordination and planning of multiple health care services; acts on the behalf of the Veteran to assure that necessary clinical services are received and monitors progress. Provides ongoing evaluation of care management services. Communicates and interacts appropriately and courteously with all internal and external customers. Maintains confidentiality of electronic, written and verbal Veteran sensitive information per HIPAA guidelines. Demonstrates practices that include adherence to Infection Control and safe use and operation of equipment. Participates in projects within the department to improve processes and constantly evaluates effectiveness of current processes and makes recommendations to improve. Work Schedule:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Experience:
Minimum of 3 years of clinical nursing experience to include but not limited to Med-Surge, Perioperative, Intensive Care Unit (ICU), Case Management or Emergency Medicine, Inpatient (discharge planning assessment and implementation of home health services, supplies, and activities required to successful transition the patient to the appropriate non-acute care setting), Outpatient (Collaboration with the Team, Veteran, Primary Care Provider, and Clinical Support Technician) and larger Team (family/caregiver, internal and community-based services involved in providing care to the Veteran). Demonstrated knowledge of InterQual Criteria, utilization management processes, clinical documentation requirements, current standards of care, and compliance guidelines. Demonstrated work experience in utilizing critical thinking skills and sound judgment to ensure positive outcomes, maximizing third-party collections and delivering quality patient care performance improvement activities. The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements as a condition of employment. The incumbent who provides care in home settings or other off-site locations may be required to drive and/or ride in GSA-vehicles.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
