Registered Nurse Manager (Patient Aligned Care Team)

The Primary Care Registered Nurse (RN) functions under the Patient Aligned Care Team (PACT) model and is responsible for the overall direction of the team members to achieve desirable patient outcomes. The PACT RN adheres to established policies, standards of care and standards of nursing practice; demonstrates leadership and serves as a role model. Serves as the Team Leader/Charge Nurse on the unit and works under the supervision of the Nurse Manager or designee. Utilizes nursing process for systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems. Provides services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well-being; and executing medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider. The nurse's primary commitment is to the patient, whether an individual, family, group, or community (ANA Code of Ethics for Nurses). The PACT RN performs the following clinical processes, responsibilities and duties:
Performs other related duties as assigned. Functions effectively as the charge nurse, team leader or in other leadership role that guides and directs others who provide patient care Demonstrates evidence of critical thinking skills in identifying, analyzing, and resolving patient care problems on the unit Performs ongoing assessment of patient condition and response to treatment Initiates and implements an interdisciplinary plan of care based on assessments Uses a team approach to identify, analyze, and resolve patient care problems that result in improved patient outcomes Facilitates care coordination and access to Specialty Care through communication and utilization of various consult types Documents accurately in the medical record of patient findings, assessments, and care provided Provides documentation in a technically informative, complete, accurate, consistent, concise and timely manner. Utilizes nursing protocols with attention to the individuality of patients, patient safety and evidence-based practice Completes patient discharge follow-up calls within seven days of discharge Demonstrates proficiency in utilizing Primary Care Data Sets to identify high risk patients and review other specific patient populations Assists with specialized Women's Health procedures Work Schedule:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Minimum 2-3 years Patient Aligned Care Team (PACT) and Women's Health Experience Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
